Një qasje e re ndaj duhanpirjes, dokumenti italian që po nxit debat në shëndetin publik

Shoqata Italiane për Patologjitë e Varësisë (SIPaD) ka publikuar një Position Statement zyrtar mbi reduktimin e riskut në çrregullimin nga përdorimi i duhanit, duke njohur për herë të parë në Itali rëndësinë klinike të strategjive me risk të reduktuar për duhanpirësit adultë që nuk arrijnë të heqin dorë nga cigaret tradicionale.

Dokumenti, i titulluar “Risk reduction in Tobacco Use Disorder (TUD): Critical analysis of the available evidence”, analizon evidencat shkencore mbi produktet pa djegie, si cigaret elektronike dhe produktet me ngrohje të duhanit (HTPs), duke i konsideruar ato si një alternativë të ndërmjetme për rastet kur abstinenca e plotë nuk është menjëherë e arritshme.

Fokus tek pacientët që nuk arrijnë të lënë duhanin
Sipas SIPaD, duhanpirja mbetet një nga shkaqet kryesore të sëmundshmërisë dhe vdekshmërisë së parandalueshme, me pasoja veçanërisht të rënda tek personat me çrregullime të varësisë dhe pacientët me probleme të shëndetit mendor. Në këto kategori, tentativa për të lënë duhanin shpesh dështojnë, ndërsa konsumi i vazhdueshëm i cigareve ndikon ndjeshëm në uljen e jetëgjatësisë dhe përkeqësimin e gjendjes shëndetësore.
Në deklaratën për mediat, presidenti i SIPaD, Claudio Leonardi, thekson se reduktimi i riskut është një qasje tashmë e konsoliduar në trajtimin e varësive dhe se ajo mund të aplikohet edhe në rastet e duhanpirjes, kur ndërprerja e menjëhershme e konsumit rezulton e vështirë.
“Objektivi final mbetet gjithmonë lënia e duhanit, por nuk mund të injorohen pacientët që duan të heqin dorë dhe nuk ia dalin”, thekson ai.

Çfarë thonë evidencat shkencore
Dokumenti përmbledh një seri studimesh toksikologjike dhe klinike mbi produktet pa djegie. Sipas SIPaD, produktet me ngrohje të duhanit funksionojnë duke ngrohur duhanin pa proces djegieje, çka redukton ndjeshëm krijimin e substancave toksike që gjenden në tymin e cigareve tradicionale.
Shoqata citon prova sipas të cilave aerosoli i prodhuar nga HTP-të përmban 92–97% më pak substanca toksike dhe potencialisht të dëmshme krahasuar me tymin e cigareve, si edhe një numër shumë më të ulët përbërësish kimikë.
Në studimet klinike, duhanpirësit që kaluan plotësisht tek alternativat pa djegie treguan ulje të ndjeshme të biomarkerëve të ekspozimit ndaj substancave toksike, përfshirë monoksidin e karbonit, nitrozaminat dhe komponimet organike volatile. Përmirësime u vunë re edhe në tregues të stresit oksidativ dhe inflamacionit.
SIPaD thekson gjithashtu se disa studime kanë evidentuar përmirësime në parametrat respiratorë dhe kardiovaskularë tek pacientët që zëvendësuan plotësisht cigaret me produkte pa djegie, përfshirë persona me sëmundje pulmonare obstruktive kronike (COPD).

“Jo pa risk” dhe jo për të rinjtë
Megjithatë, dokumenti nënvizon qartë se produktet alternative nuk janë pa risk, nuk konsiderohen medikamente dhe nuk duhet të promovohen tek jo-duhanpirësit apo të rinjtë.
SIPaD kërkon që çdo komunikim publik mbi këto produkte të jetë i kujdesshëm, i balancuar dhe pa karakter promovues, duke shmangur çdo normalizim të përdorimit të nikotinës.
Shoqata thekson se strategjitë me risk të reduktuar duhet të konsiderohen të përkohshme, të monitoruara dhe pjesë e një rruge terapeutike të strukturuar që synon, në fund, ndërprerjen e plotë të konsumit të duhanit.

Një debat që po fiton terren në Europë
Publikimi i kësaj deklarate vjen në një moment kur debati mbi reduktimin e dëmit nga duhani po fiton gjithnjë e më shumë vëmendje në Europë dhe në komunitetin ndërkombëtar të shëndetit publik.

Ndërsa organizatat shëndetësore vijojnë të theksojnë se alternativa më e mirë mbetet mosnisja ose lënia e plotë e duhanit, një pjesë e ekspertëve argumentojnë se për duhanpirësit adultë që nuk arrijnë të heqin dorë nga cigaret, strategjitë me risk të reduktuar mund të përfaqësojnë një opsion pragmatik për uljen e dëmit dhe të ekspozimit ndaj substancave toksike.

A New Perspective on Smoking: The Italian Document Fueling Debate in Public Health

The Italian Society for Addiction Diseases (SIPaD) has published an official Position Statement on risk reduction in Tobacco Use Disorder (TUD), formally recognizing for the first time in Italy the clinical relevance of reduced-risk strategies for adult smokers who are unable to quit traditional cigarettes.

The document, titled “Risk Reduction in Tobacco Use Disorder (TUD): Critical Analysis of the Available Evidence”, reviews the scientific evidence surrounding smoke-free alternatives such as e-cigarettes and heated tobacco products (HTPs), considering them an intermediate option for cases in which complete abstinence is not immediately achievable.

Focus on smokers who cannot quit
According to SIPaD, smoking remains one of the leading preventable causes of disease and death, with particularly severe consequences among people with substance use disorders and patients with mental health conditions. In these populations, smoking cessation attempts often fail, while continued cigarette consumption significantly reduces life expectancy and worsens overall health outcomes.
In comments released to the press, SIPaD President Claudio Leonardi emphasized that harm reduction is already a well-established approach in addiction treatment and may also be applied to smoking when immediate cessation proves difficult.
“The final goal must always remain smoking cessation, but we cannot ignore patients who want to quit and are unable to do so,” he stated.

What the scientific evidence says
The document reviews a range of toxicological and clinical studies on smoke-free products. According to SIPaD, heated tobacco products work by heating tobacco without combustion, significantly reducing the generation of toxic substances found in cigarette smoke.
The society cites evidence showing that aerosol generated by HTPs contains 92–97% fewer toxic and potentially harmful substances compared to cigarette smoke, as well as a substantially lower number of chemical compounds overall.
Clinical studies referenced in the document show that smokers who completely switched to smoke-free alternatives experienced significant reductions in biomarkers associated with exposure to toxic substances, including carbon monoxide, nitrosamines, and volatile organic compounds. Improvements were also observed in markers linked to oxidative stress and inflammation.
SIPaD further highlights that several studies have shown improvements in respiratory and cardiovascular parameters among patients who fully replaced cigarettes with smoke-free products, including individuals with chronic obstructive pulmonary disease (COPD).

“Not risk-free” and not for young people
At the same time, the document clearly underlines that alternative nicotine products are not risk-free, are not medicines, and should not be promoted to non-smokers or young people.
SIPaD stresses that all public communication around these products must remain cautious, balanced, and free from promotional intent, avoiding any normalization of nicotine use.
The society also states that reduced-risk strategies should be considered temporary, carefully monitored, and integrated into a structured therapeutic pathway ultimately aimed at complete smoking cessation.

A debate gaining momentum across Europe
The publication of this Position Statement comes at a time when the debate around tobacco harm reduction is receiving growing attention across Europe and within the international public health community.

While health organizations continue to emphasize that the best option remains never starting smoking or quitting entirely, a growing number of experts argue that for adult smokers who are unable to quit cigarettes, reduced-risk strategies may represent a pragmatic option to lower harm and reduce exposure to toxic substances.
